OIG Reports on DNFSB

The Office of the Inspector General (OIG) provides DNFSB employees, other Government employees, contractor employees, and the public with a confidential means of reporting incidents of suspicious activity concerning fraud, waste, abuse, and employee or management misconduct. Individuals are also encouraged to report concerns regarding mismanagement of agency programs or dangers to public health and safety.

People may contact OIG by telephone, online, or by mail.   OIG does not attempt to identify persons contacting the Hotline, and no identifying information is captured when you submit an online form. You may provide your name, address, or phone number, if you wish.

What should be reported?

Indications of the following:  Management/supervisory retaliation; contract and procurement irregularities; conflicts of interest; theft and misuse of property; travel fraud; misconduct; time and attendance abuse; misuse of Government credit card; abuse of authority; program mismanagement; misuse of information technology resources; substantial and specific danger to the public health and safety; computer crimes; and other violations of Federal laws and regulations.

What information should be provided?

A brief, accurate statement of facts believed to provide evidence of wrongdoing; names, addresses, and office locations of pertinent individuals and organizations; dates and times when the suspected wrongdoing took place or is expected to occur; how you became aware of the information; and memoranda, contracts, invoices, or other related documents.
